Welcome to our office! In order to make things run smoother, please download the New Patient Paperwork and the Office policy sheet, fill out all the necessary information and bring it to your first visit. You will find in the New Patient Paperwork the following items:

1. New Patient Information

2. Health History Form

3. Dental Materials Fact Sheet (DMFS)- this explains all the materials used in the dental field.

4. HIPPA- this is the health information privacy protection form

5. DMFS & HIPPA acknowledgment form- please sign and date that you received these forms

6. Office policy- this describes our policy on financial options, please read it carefully and sign it    and bring it to your first appointment.
